Scheduling your First Appointment

Record RequestYour doctor at East Bay Pediatrics will be better able to care for you and your child if medical records from your previous physician are available. Patient visit summaries which you have access to may not provide enough information for your doctor. Please complete the Record Request Form and send to your previous pediatrician so that official medical records can be sent to our office before your first visit.

First Well AppointmentYour first appointment is a time for your doctor to get to know you and your child. We will spend a little extra time learning about both your child’s health history and life situation. We believe that knowing the whole family helps give comprehensive care to the child.
The visit will include a review of both the child’s and the family’s medical history. Please try to have previous medical records sent to the practice ahead of time. Bring a list of your child’s medications, doses, and whether refills are needed; let us know what pharmacy you prefer for electronic prescribing. A complete physical exam will be done as well as a review of vaccinations. Our medical assistants give vaccinations in the same room as the exam. Sometimes routine screening procedures occur during the visit as well. Make sure to schedule your next appointment on your way out of the office! Please note that we at East Bay Pediatrics feel strongly that vaccines protect children from many severe illnesses.
