Your First Appointment
Call (925) 438-1100 & choose option 8 (Hours: 9:00am-12:00pm and 2:00pm-5:00pm) to speak with one of our New Patient Coordinators to schedule your first appointment. If your child has an urgent issue, she may transfer you to the phone advice nurses to schedule your appointment.
Please bring the following to your first appointment:

Please bring the following to your first appointment:
- Immunization Record
- Medical records from your previous physician. If you do not have records, please complete this Record Request Form and send it to your previous physician so that your records can be forwarded directly to our office.
- New Patient Questionnaire
Please try to complete prior to your first visit so that your doctor can review the information with you at the first visit. (NOTE: only complete if your child is 2 years or older) - Consent Form
One of these forms needs to be signed for each family in order for our physicians to care for your child in the event that someone other than yourself needs to bring your child to our office, e.g. a grandparent or a nanny. - Patient Information Form
One of these forms should be completed for each child's chart - Patient Portal Form
Complete this form for access to your child's medical record online or on your smartphone. This form is also required for login access to email your doctor.
