Scheduling your First Appointment

  1. Please start by completing the New Patient Intake Form and our New Patient Coordinator will contact you once they have created your account.  You can follow up by calling (925) 438-1100 & choose option 6  and leave a message if you need additional assistance. If your child has an urgent issue, they may transfer you to the phone advice nurses to schedule your appointment. 
  2. Once you have emailed the New Patient Intake Form back, the coordinator will create your account  and contact you to schedule your appointments.

Record Request

Your doctor at East Bay Pediatrics will be better able to care for you and your child if medical records from your previous physician are available. Patient visit summaries which you have access to may not provide enough information for your doctor. Please complete the Record Request Form and send to your previous pediatrician so that official medical records can be sent to our office before your first visit.

First Well Appointment

Your first appointment is a time for your doctor to get to know you and your child. We will spend a little extra time learning about both your child’s health history and life situation. We believe that knowing the whole family helps give comprehensive care to the child. 

The visit will include a review of both the child’s and the family’s medical history. Please try to have previous medical records sent to the practice ahead of time. Bring a list of your child’s medications, doses, and whether refills are needed; let us know what pharmacy you prefer for electronic prescribing.

A complete physical exam will be done as well as a review of vaccinations. Our medical assistants give vaccinations in the same room as the exam. Sometimes routine screening procedures occur during the visit as well.

Vaccinations
Please note that we at East Bay Pediatrics feel strongly that vaccines protect children from many severe illnesses. To learn about our vaccine policy and our minimal vaccine requirements, please see our Vaccine Philosophy page.

Please bring the following to your first appointment:

  • Immunization Record
  • Medical records from your previous physician. If you do not have records, please complete this Record Request Form and send it to your previous physician so that your records can be forwarded directly to our office.
  • New Patient Questionnaire
    Please try to complete prior to your first visit so that your doctor can review the information with you at the first visit. (NOTE: only complete if your child is 2 years or older)
  • Consent Form
    One of these forms needs to be signed for each family in order for our physicians to care for your child in the event that someone other than yourself needs to bring your child to our office, e.g. a grandparent or a nanny.
  • Patient Information Form
    One of these forms should be completed for each child's chart
  • Patient Portal Form
    Complete this form for access to your child's medical record online or on your smartphone. This form is also required for login access to email your doctor.

We schedule additional time for your first appointment so that the doctor can learn more about you and your child as well as tell you about the practice. If you have any special considerations, please call our new patient coordinator at (925) 438-1100 & choose option 6.
