UCSF Clinically Integrated Network

East Bay Pediatrics will join the UCSF Clinically Integrated Network (CIN) starting May 19, 2026.
​
We at East Bay Pediatrics are excited to partner with UCSF while continuing to run this pediatric practice ourselves. This is the only partnership which allows us to remain an independent business, running operations according to our own doctors' decisions. 

What does this mean for you? 

  • Same excellent care: No change to our doctors or staff.
  • Same phone advice nurses answering your questions and scheduling appointments.
  • Same ability to have referrals with any specialist, whether with UCSF, Stanford, Sutter, independent, or other specialist.
  • Shared clinical quality with UCSF CIN pediatric practices. Data but no names or other personal data will be shared.
  • Access to patient charts though UCSF MyChart *see instructions below
  • Updated privacy policies for patients 12 years and up (see below for more)
  • Updated billing through UCSF Health Medical Foundation *for visits on May 19 and after
  • Same insurance plans accepted - and more! all the same PPOs plus UCSF Health includes Hills Physicians HMO in addition to Brown & Toland HMO​

What can you do now for a smooth transition?

  • Pay your bill online with the old patient portal. 
    • Visits prior to May 19 are being billed to your insurance through PedsOne. Save yourself confusion by paying your bill now
  • Sign up for MyChart with the new UCSF electronic health record.  
    • **Start in "What should you do next?" section below if: 1. you already have a MyChart account, 2. if the patient is over 18 years old, or 3. if you have an upcoming appointment
    • First, set up a parent account
      • On this website: MyChart | UCSF Health choose “Signup without an Activation code”
      • You will be asked to verify your identity through Experian. 
    • Once you have created a MyChart account, you can request proxy access to your child's chart.  
      • Profile > Account Settings > MyChart Proxy Center - you will gain access in about 2-3 business days (make sure that you carefully enter your child's name and date of birth!)
      • *see Privacy conditions for various age groups  - scroll down to the section called UCSF MyChart for My Family
    • Make sure to download the MyChart app on your cellular phone
    • If you are unable to login after completing this process, call the MyChart help desk at 415-514-6000

What should you do next? 

update your demographics (yes - you have to do this again)
Speed up your check in process by completing this form before your appointment.
If you have a MyChart account, you will be able to do this as a pre-Registration step before your upcoming appointment.

Having these documents completed and signed will make it easier to check in for your already scheduled appointment and easier  to make a sick or urgent appointment at East Bay Pediatrics.
After May 19, you can bring these documents to your appointment or mail them in. Once you have a MyChart account (see below) you will be able to complete these forms online during the pre-check in process.

​Click on the above button to access the Terms and Conditions of Registration, Medical Services and Financial Agreement . This document is required for any health services through East Bay Pediatrics. 
Consent to Treatment of Minor
The Consent to Treat form allows East Bay Pediatrics to care for your child in an emergency. The bottom part of the form includes space where you can designate caretakers other than parent or legal guardian to bring the child for treatment.

Please review the Notice of Privacy Practices then download and sign the Notice of Privacy Practices Acknowledgement. The QR code at the bottom of the form will take you to the full HIPAA privacy policy. You can also learn more about privacy on the About EBP page under Office Policies.

It is recommended that each person 12 and over has his/hers/their own MyChart account. See below for how to access MyChart for each age group. Parents can become a proxy to be able to view their children’s charts. Please choose the right age group to learn how. If you are already in the office, you can request an activation code to sign up more quickly. ​

Sign up for MyChart
Already have a patient UCSF MyChart account:
  • This may apply to you if the patient has seen a specialist; had a procedure, labs or Xrays; or been to the emergency room at a UCSF facility
  • Login to MyChart and confirm that your East Bay Pediatrics doctor is listed as the patient's Primary Care Physician:
    • Use the menu on the top right to choose the patient's name
    • Click on the My Health Care Team button and confirm that your pediatrician is listed as the Primary Care Provider. 
    • If your pediatrician is not listed, you will need to call our office or request an activation code in our office. 
  • Download MyChart App if you have not already done so
  • Update your demographics before your appointment (see below)
Sign up with an activation code: Required for 18+
If you are in the office OR talking with a staff member on the phone, you can request an activation code to simplify the MyChart sign up process. 
  • Instead of using Experian,  our staff will verify your name, phone number and date of birth
  • Staff can then send an Activation code to you by text or give you a printed activation code. 
Send us a message
  1. Login to MyChart on your phone or computer.
  2. Make sure to toggle the screen so that you are in the patient's chart (parents or guardians are proxies and cannot message us through their own account)
  3. Click Create a Message. ​​​
***Remember that messages sent via MyChart should be NON-urgent. We will respond to you in 2-3 business days. 

Review your patient chart
View your After Visit Summary
  • Click on Encounters and choose the correct appointment date/time
  • Click on View After Visit Summary
  • Any instructions from the visit will be seen here. 
View your Labs or Results
  • Click on Results to view any recent labs or radiology procedures. 
  • Remember that results may come to you immediately. Your doctor may not see results if he/she is seeing patients or not in the office that day. If you have not heard from us about your results after 1 week, please give us a call 925-438-1100

Adult patients need to complete registration themselves. All three of these forms are required for care with East Bay Pediatrics

Complete the Registration form. Make sure to use the phone number and email where you want appointment reminders and lab results to go. 
Terms and Conditions
Sign the Terms and Conditions which is required for health services through East Bay Pediatrics. ​
Notice of Privacy Practices

A MyChart Account is strongly recommended. This will allow you to view your medical record, communicate with your doctor and share health information with your family.
UCSF MyChart
​**You will need an activation code to sign up for MyChart. As a young adult, you will need to contact our office to request your code. You can also do this if you are in the office for an appointment. 
Release of Health Information
Communication Form
As an adult, your health information is private from other people, including your parents. Sign the Release of Information form to allow East Bay Pediatrics to communicate with your parents about appointments, test results, treatment plans, etc. The Communication form allows for more detailed consent.
