Choosing a new pediatrician can be a confusing task. You may be seeking a new pediatrician because of a changes in insurance coverage, relocation, or simply needing a better "fit." Our practice offers a variety of services including weekend and after hours coverage, in-office procedures, and an experienced staff. Our physicians, while stemming from various backgrounds, have a similar approach to medicine and work together toward the best health care for your child.

Your First Appointment
Our New Patient Coordinator schedules your first appointment. Call (925)438-1100, option 8. If your child has an urgent issue, she may transfer you to the phone advice nurses to schedule your appointment.
Please bring the following to your first appointment:
- Immunization Record
- Medical records from your previous physician. If you do not have records, please complete this Release of Information Form and send it to your previous physician so that your records can be forwarded directly to our office.
- New Patient Questionnaire – Please try to complete prior to your first visit so that your doctor can review the information with you at the first visit.
- Consent Form One of these forms needs to be signed for each family in order for our physicians to care for your child in the event that someone other than yourself needs to bring your child to our office.
- Patient Information Form One of these forms should be completed for each child's chart
We schedule additional time for your first appointment so that the doctor can learn more about you and your child as well as tell you about the practice. If you have any special considerations, please call our new patient coordinator at (925)438-1100, option 8.
